Case management – Clients with loss of autonomy

This “tailor-made” training enables participants to deepen their appreciation of the concepts of integration, coordination, continuity of services and case management for clients with loss of autonomy.

Participants gain understanding about the role of the case manager and the impact of his or her actions on the quality of services offered to this clientele.

The participation of directors of adult residential care facilities as well as health and social services managers in the training is recommended since certain organizational aspects necessary to successful case management are discussed.
